I grew up in New England and things were a bit more casual in our family. We called our aunts and uncles by their first names -- that is, "Joanne", rather than "Aunt Joanne" -- and all of my cousins did the same with my parents. We also called some of our parents' friends by their first names, but mostly when referring to them, not speaking directly to them. I'm really surprised that my parents were fine with that. But we would never have dreamed of calling grandparents by their first names.
